docs(telemetry): update Phase 3/4 task lists for SpanGuard factory pattern

Browse Source 
Replace references to old XRPL_TRACE_TX/CONSENSUS macros with
SpanGuard::span(TraceCategory, ...) factory calls introduced in Phase 1c.
