The document is internal and is kept outside the repo instead. This also removes every link to it: the document-index rows in OpenTelemetryPlan.md and 08-appendix.md, its node, edge and style lines in the plan's Mermaid map, its own section in OpenTelemetryPlan.md, and the two cross-reference notes in 02-design-decisions.md and 05-configuration-reference.md.
