Nik Bougalis 8f58687091 Cleanup AcceptedLedger and AcceptedLedgerTx:
This commit modernizes the `AcceptedLedger` and `AcceptedLedgerTx`
classes, reduces their memory footprint and reduces unnecessary
dynamic memory allocations.
