* Defining RIPPLE_PROPOSE_FEATURES enables ledger features
* Reduce dependence on getApp via dependency injection
* Converted uint32 members to use Clock::time_point
* Inject Journal, tidy up classes:
    - Features
    - FeaturesImpl
    - FeeVote
    - FeeVoteImpl
