Various cleanups:

Browse Source 
* Replace SYSTEM_NAME and other macros with C++ constructs
* Remove RIPPLE_ARRAYSIZE and use std::extent or ranged for loops
* Remove old-style, unused offer crossing unit test
* Make STAmount::saFromRate free and remove default argument
