Refactor RippleCalc.cpp:

* Add comments
* Restrict code to 80 colums
* Remove boost::format
* Remove BOOST_FOREACH
* Make members private
* Add ripple_unordered_set to UnorderedContainers.h
* Replace boost::unordered_set with ripple::unordered_set
